Notre objectif est de découvrir les mécanismes physiopathologiques de la douleur un problème majeur de santé publique avec un impact à la fois sur le patient et la société. Pour aborder cette thématique, nous avons développé une stratégie de recherche translationnelle inverse, partant du lit du malade à la paillasse et vice versa : c’est à dire, un problème clinique étant identifié, ses mécanismes sont étudiés sur des modèles animaux appropriés ; si de nouvelles cibles cellulaires ou moléculaires sont découvertes ou de nouvelles explorations développées, elles sont validées cliniquement par retour au lit du malade. Ainsi, notre recherche fondamentale reste-elle toujours cliniquement fondée. De plus, notre ancrage dans le monde socio-économique facilite la conception de nouveaux équipements et/ou traitements pour améliorer la prise en charge des syndromes douloureux.

Notre unité est constituée de deux équipes qui partagent leurs compétences et expertises, se renforçant ainsi mutuellement à de multiples niveaux: projets (collaboratifs), ressources humaines, savoir-faire et idées, approches techniques et moyens. L’intérêt de l’équipe 1 se porte principalement sur la physiopathologie et la pharmacologie des douleurs chroniques extracéphaliques avec l’étude des mécanismes d'action et modalités de prescription des médicaments analgésiques actuels et l’exploration de nouvelles stratégies dans le traitement des douleurs chroniques. L’équipe 2 étudie les douleurs trigéminales (céphalique et orale), se focalisant plus particulièrement sur la migraine et l’hypersensibilité cutanée. Ces deux équipes collaborent sur des projets à la frontière entre leurs deux thématiques « douleurs céphaliques et extracéphaliques ». L'ensemble de nos travaux a permis (i) le développement de nouveaux outils pour explorer le fonctionnement du cerveau dont: l’optogénétique, l’imagerie calcique, la microscopie multi-photonique, le système ultrason pour analyser la connectivité fonctionnelle du cerveau, les outils mathématiques pour analyser des données obtenues chez l’homme comme chez l’animal; (ii) le développement de projets de recherche nouveaux et originaux:  processus cérébraux de la  douleur (équipes 1 et 2), pharmaco-épidémiologie de la  douleur et sa relation avec le microbiote (équipe 1), changements fonctionnels et anatomiques du néocortex chez le migraineux (équipe 2), relations entre migraine et hyperacousie (équipes 2); et (iii) l’amélioration de notre visibilité et attractivité internationale.

Our goal is to discover the pathophysiological mechanisms of pain and hearing disorders, two major public health problems of great impact on both the patient and society. Of note, to address such clinical issues, we have developed a reverse translational research strategy that goes from bedside to bench and vice versa; that is, a clinical problem being identified, its mechanisms are investigated using appropriate animal models; if novel cellular or molecular targets are revealed or new explorations developed, they are then clinically validated by getting back to bedside. Therefore, our basic research is always clinically relevant. Moreover, because of our close interactions with socio-economical partners, it could easily lead to designing new equipment and improving therapy to help control such pain and hearing disorders.

Our unit results from the merge of three teams who share their expertise and strength, thus getting potentiated at multiple levels: (collaborative) projects, human resources, skills and ideas, technical approaches and means. Team’s 1 main interest is on the pathophysiology and pharmacology of chronic pain, including the study of mechanisms of action and prescription patterns of current analgesic drugs and the design of new strategies in the treatment of chronic pains. Team 2 investigates trigeminal (cephalic and oral) pain, focusing on migraine and mechanical allodynia. Team 3 develops tools for measuring auditory performance and its modulation, e.g. by intracranial pressure, thus extending its field of interest to extra-auditory domains in relation to pain. In addition, the three teams collaborate on specific projects between on the one hand chronic spinal and trigeminal pain and on the other handpain and hearing. Our merger boosted (i) the development of new tools for investigating brain functions including: optogenetics, Ca2+ imaging, multi-photon microscopy, ultrasound system to analyze brain functional connectivity, mathematical tools to analyze data obtained from humans as well as animals; (ii) the opening of new research avenues including: brain processes in pain (team 1 and 2), pharmaco-epidemiology of pain and its relationship to microbiota (team 1), functional and anatomical changes in migraine neocortex (team 2), relationships between migraine and hyperacusis (teams 2 and 3); and finally (iii) our international visibility, the best way to attract bright new people.
